For years I’ve had a constant and stubborn tightness on my hips and lower back. After many trips to the physiotherapist and countless exercises I was introduced to Theramax. During first session not only did I feel an immediate release on my hip flexors but at the end the pain in my lower back and sciatica appeared to have decreased tremendously. The specialist also gave me a list of at home exercises that I’ve been diligently incorporating in my workouts. Looking forward to my next session! Thank you to the team at Theramax, you are onto something!

How Piriformis Syndrome Disrupts Life in Miami
Sitting becomes torture: Your buttocks burn like crazy the moment they touch a chair. Dinner at Garcia's Seafood means constantly shifting positions. The drive down Biscayne Boulevard is unbearable. You can't work, can't drive, can't function as normal.
Work has stopped completely: You can't sit at a desk. Eight weeks off work because sitting triggers 10/10 pain. Even the drive to Lincoln Road leaves you gripping the wheel, counting the minutes until you can stand again.
It's sucked the joy out of your life: You're crawling around the house instead of walking. Days at Zoo Miami feel impossible. That constant ache, like a toothache that never lets up, has you wondering if this is just how life is now.
South Florida residents don't have to live this way.
The piriformis is one of the hardest muscles to reach.
It sits beneath three layers of gluteal muscle. That's why stretching barely touches it, tennis balls often make it worse, and PT hands fatigue before getting deep enough. Many treatments fail because they can't access where the problem actually lives.
Piriformis Syndrome Relief at TheraMax
Piriformis syndrome happens when your piriformis muscle, buried deep beneath your glutes, becomes chronically tight and may compress your sciatic nerve. This creates burning buttock pain and radiating leg pain that flares the moment you sit. The problem is depth. Traditional methods rarely can get through the thick gluteal tissue needed to target the piriformis. That's where robotic therapy comes in. Sustained pressure at depths manual therapy can't reach.
Clients often report they can:
Return to work and drive without burning buttock pain stopping them
Sit through dinner, meetings, and daily activities comfortably again
Sleep through the night and return to walks, exercise, and active living

What Makes Our Approach Different
Swipe to compare treatment options
Chiropractor
Approach: Hip and spine adjustments with soft tissue work on piriformis
Timeframe: Multiple visits over weeks/months
Effectiveness: May provide temporary relief when alignment is off
Adjustments focus on spinal alignment, not the deep gluteal tissue where piriformis compression occurs. When the spine is fine but the muscle stays tight, chiropractic care often provides only temporary relief.
Physical Therapy
Approach: Stretching exercises and manual therapy targeting the piriformis muscle
Timeframe: 8-12 weeks of sessions
Effectiveness: Can help mild cases but limited by depth and sustained pressure needed
PT stretches can help, but therapist hands fatigue quickly and can't maintain the sustained deep pressure chronic piriformis tightness requires. Many clients have done months of PT with limited lasting improvement.
Injections & Surgery
Approach: Steroid injections into piriformis or surgical muscle release
Timeframe: Recovery time varies by procedure
Effectiveness: Injections reduce inflammation temporarily; surgery has risks and variable success
Injections reduce inflammation temporarily, but they wear off in weeks or months. They don't address the tight piriformis muscle contributing to compression. Surgery is irreversible and recovery can take months.
Understanding Piriformis Syndrome
The piriformis sits directly on top of your sciatic nerve
Your piriformis is a small, deep muscle that runs from your sacrum to your hip bone, passing directly over your sciatic nerve. In some people, the nerve actually runs through the muscle. The nearby iliopsoas, your deep hip flexor, can also contribute when chronically tight from sitting. When these muscles become chronically shortened, they can compress the nerve and create burning buttock pain that radiates down the leg. This explains why back-focused treatments may fail.
Frequently Asked Questions
Get answers to your questions about robotic therapy
I've tried PT, stretching, and even tennis balls. Nothing works. Why should I try this?
Because we reach what those treatments can't. The piriformis sits beneath thick gluteal tissue. PT hands fatigue. Tennis balls often make it worse. We deliver sustained pressure at depths manual therapy can't safely access.
I can't sit through a meeting or drive down Biscayne Boulevard without burning pain. When will this end?
Piriformis syndrome can be stubborn, especially if it's been building for months. Some clients improve in the first few sessions. Others need more time. We can't promise a timeline, but we'll give you an honest assessment after your first visit.
The pain never stops. It's like a constant toothache in my buttock, and I can't sleep.
That constant ache is exhausting. Tight muscles can stay active around the clock, making sleep miserable. As tension releases, sleep is often one of the first things that improves.
My doctor recommended steroid injections. Should I try those first?
Many clients come to us to avoid injections. They typically help for weeks or months, then wear off because they don't address the tight muscle. Some prefer non-invasive options first. We recommend discussing with your doctor.
Is this going to keep getting worse? I feel like I've lost all hope.
Piriformis syndrome is muscular, not degenerative like arthritis. Once the muscle releases, improvement is possible. Many clients who came to us ready to give up are back to walking Tropical Park and driving to Coral Gables without constant pain.
